This post carries continuing responsibility for pro-active management of a caseload of individuals identified as having complex health and social care needs, with a diagnosis of MS. This post also includes the support and education of those individuals newly diagnosed with MS.
The active management of these individuals will have the specific aim to support them and their families to develop self care strategies and to facilitate a pro-active management plan of their health condition, with the intent to reduce inconsistencies in the care they receive, plan interventions for fluctuations in health status and to prevent inappropriate hospital admission.
You will work within a team of other Community Matrons (including another MS nurse and MS OT) in a manner which ensures continuity of service for all patients, supporting each other and patients to meet the aims of the service.
